As a nurse, Founder Janna Gardner understood the critical difference that quality medical care can make in someone’s life. Healing Hands is a patient-focused community health center capable of caring for a patient from birth to the end of their life. Healing Hands Ministries focuses on empowering the community by improving access and delivery of healthcare, providing education to help maintain improved lifestyles and inspires others to take responsibility for their own health. Today, Healing Hands Ministries has rebranded to HHM Health, beginning an exciting new chapter of expansion. As an FQHC, HHM Health offers family practice, women’s health, pediatrics, dental, vision and behavioral health services to patients regardless of ability to pay. HHM Health is dedicated to delivering cost-effective, high-quality medical services to residents of Dallas County, Texas, guided by their mission of providing healthcare to all with love, compassion, and respect. The organization will utilize assistance from the Health Resources and Services Administration (HRSA) and the Ryan White HIV/AIDS Program (RWHAP) to enhance its ability to adapt to the evolving healthcare landscape and improve access to top-notch HIV primary care for underserved, minority, and low-income individuals living with HIV. HHM Health will emphasize innovation in HIV care, incorporating behavioral health into HIV primary care. HHM Health will strive to achieve the following goals: - Develop polices and procedures to facilitate a caring and factual response to HIV+ test results - Develop a protocol for rapid screen followed by more-in-depth HIV testing - Train staff to understand best practices for trauma-informed care to relaying HIV+ results to a patient - Clarify workflow, responsibilities and documentation - Create a sustainable tracking process for HIV+ results, linkage to care, behavioral health services, etc. By the end of project funding, HHM Health will have expanded its organizational capacity to reach more patients throughout the Dallas community, targeting at-risk populations and ensuring these populations are aware of the services offered by HHM Health. HHM Health will create a foundational sustainability plan that will be achieved through quality improvement activities, that will establish a plan for the continuous growth and expansion of programming beyond grant funding, serving the identified patient population for years to come.
