Ryan White Title III HIV Capacity Development and Planning Grants - Dallas County Health and Human Services, recipient for the Ryan White Part A Dallas Eligible Metropolitan Area (EMA) (DCHHS, Applicant) (UEI Number ER74JB3UL5E9), is the Applicant under Health Resources and Services Administration Notice of Funding Opportunity HRSA-23-052, Ryan White HIV/AIDS Program Part C Capacity Development Program. With this application, the Applicant is requesting funding for an Infrastructure Development activity, the implementation of Telemedicine Technology and related technology tools to strengthen its organizational capacity to respond to the changing healthcare landscape and to enhance and increase access to high-quality HIV primary healthcare services for low income, uninsured, and underserved people with HIV living in the EMA. The proposed Telemedicine Project is designed to address a number of needs and barriers to providing HIV care in the Dallas EMA, which have been identified by the Applicant and the Ryan White Planning Council of the Dallas Area, and to address barriers to client linkage, outreach and retention in care created by the COVID-19 pandemic, which has adversely affected access to HIV care the EMA. The proposed Project has three goals, which correspond to the three Project components: 1. Customization and Implementation of the e2MyCare App. The e2MyCare App is a user-friendly Telehealth tool accessible from any device connected to the internet, including smartphones and other mobile devices, which will provide clients with secure, remote access to a number of features specifically designed for HIV patients, to support client engagement and retention in care. 2. Customization and implementation of the e2Linkage to Care Module and Report. The e2Linkage to Care Module and Report are a set of electronic tools for HIV education, client outreach, and linkage to care. 3. Customization and Implementation of the Client Snapshot. The Client Snapshot is a document summarizing key information about the client, including client name, demographic information, insurance information, eligibility, the client’s care team, and a summary of medical information, for use by the Applicant and subrecipient staff in situations where only key information is needed, and review of the entire client record would be unnecessary and time-consuming. The proposed telehealth Project is designed to address several needs and barriers to providing HIV care in the Dallas EMA, which the Applicant has identified and the Ryan White Planning Council of the Dallas Area (Planning Council), including (1) the need to increase access to care through telehealth as a strategy for linkage to care and retention in care for MSM and African American men and women, (2) the need to reduce administrative burden on clients by streamlining the submission of eligibility documentation, (3) a lack of HIV care in suburban and rural areas, and (4) the need for greater community outreach and education, including access to information about providers and HIV-related services available in the EMA.
