SNCHC is a Federally Qualified Health Center (FQHC), and offers family planning, Ryan White, primary health, behavioral health, and refugee health services. The purpose of the proposed activity is to improve care coordination and integration of HIV primary care with behavioral health services through the development of strategies, policies, and procedures. This activity is not an expansion of a previously funded RWHAP activity. Our Ryan White clients need behavioral health support as they learn to live and thrive with a potentially fatal chronic disease. Mental health conditions often effect medical conditions, and vice versa, so it’s important to provide whole-person care to these patients. Based on a shortage of providers and the disproportionate HIV case count in Southern Nevada, it is vital that we close the service gap for people living with HIV in Southern Nevada. The project will focus on creating organizational strategies, policies, and procedures for integrating SNCHC Ryan White services with behavioral health. An LCSW and a Psychiatric/Mental Health APRN will lead this effort. An Administrative Assistant and a Medical Assistant will be hired to support this work. The goal of the project is to seamlessly connect Ryan White patients to affordable behavioral health services.
