HOPES currently provides care to 1,036 people living with HIV/AIDS (PLWHA), approximately 665 of whom are at risk of poor HIV treatment adherence due to untreated mental health or substance use concerns. HOPES is a Federally Qualified Health Center (FQHC) located in Reno, Nevada. HOPES’ service area consists of 14 of the 17 counties in Nevada. Since 1997, HOPES has been the only comprehensive HIV provider in northern Nevada. For 25 years, HOPES has provided medical, pharmaceutical, case management, and support services to persons living with HIV/AIDS (PLWHA). In recent years, HOPES has added behavioral health, psychiatry, medication assisted treatment for opioid users, radiology, transportation, medication delivery, a breast health program, a syringe services program, a financial literacy/education program, Medical-Legal partnership and transitional housing to its list of services offered. The target population includes PLWHA who are experiencing mental health and/or substance use concerns that impact their ability to be retain in care and/or achieve viral suppression. If funded, HOPES will utilize one full-time Behavioral Health (BH) Integration provider who is licensed to conduct treatment for both mental health and substance use (cooccurring) concerns to create a formal HIV-BH integration program that identifies PLWHA at-risk of poor HIV treatment adherence and/or medical outcomes due to untreated BH concerns and conducts targeted assessments, brief BH counseling, crisis intervention, and referral to ongoing treatment (TX) during HIV medical appointments. The dedicated BH Integration (BH-I) provider will create Policies and Procedures, as well as workflow processes that enables the agency to identify and provide BH TX to PLWHA in the HIV medical clinic setting, as well as ensures follow-up and linkage to ongoing BH treatment for those with BH concerns that affect their ability to engage and adhere to HIV TX. Along with creating workflow processes that identify and provide services to PLWHA at-risk of non-HIV adherence due to BH concerns, the BH-I provider will creates processes that work to engage PLWHA in need of BH TX but are resistant to participation. Through this project HOPES seeks to achieve an 85% Retention in Care and 90% viral load suppression rate among program participants.
