Ryan White Title III HIV Capacity Development and Planning Grants - In response to the funding opportunity announcement, HRSA-22-019, from the U.S. Department of Health and Human Services Health Resources and Services Administration for the Ryan White HIV/AIDS Program Part C Capacity Development Program, Community Health Project, Inc. d/b/a Callen-Lorde Community Health Center (Callen-Lorde) proposes to develop enhanced strategies, policies, and procedures to improve care coordination and integration of HIV primary care with behavioral health services for low income, uninsured and underinsured PLWH residing within the five boroughs (Manhattan, Brooklyn, Queens, the Bronx and Staten Island) of New York City, with an emphasis on reaching and serving Black and Latinx men who have sex with men (MSM) and transgender women aged 25 and older, who are encountering barriers to retention in quality HIV primary care services, and maintaining viral suppression. The proposed activity of Integration of HIV Primary Care with Behavioral Health services falls under the HIV Innovation Category. As a provider of high-quality health care targeted to NYC’s lesbian, gay, bisexual, transgender, and queer (LGBTQ) communities, including persons living with HIV/AIDS (PLWHA), and welcoming to all regardless of ability to pay, Callen-Lorde is dedicated to addressing community needs and expanding access to HIV prevention, care, and treatment.
