Puerto Rico Childhood Lead Poisoning Prevention and Surveillance of Blood Lead Levels in Children - In September 2021, a surveillance for blood lead levels (BLLs) among children d16 years old was implemented in Puerto Rico to collect data on all blood lead tests and investigate those cases with levels above the blood lead reference value (e3.5 �g/dL). The surveillance system received BLL test reports through the National Electronic Disease Surveillance System (NEDSS) Base System (NBS). Telephone interviews were conducted to collect sociodemographic and clinical information and to determine possible sources of lead exposure for children with a BLL e3.5 �g/dL. During September 2021 through June 2023, 25,872 blood lead tests were reported. A total of 123 (0.47%) children were identified with BLLs e3.5 �g/dL; 67 (54%) were male and median age was 3 years. Among the 123 children, 64 (52%) had confirmed blood levels in the 3.5-4.9 �g/dL range, 47 (38%) had levels in the 5-10 �g/dL range, and 12 (10%) had levels in the >10 �g/dL range. The Fajardo health region reported the highest rate of childhood lead poisoning among children <5 years with 40 cases per 10,000 children. Occupations and hobbies related to lead and living in a building built before 1978 were the most reported sources of lead exposure. Blood lead surveillance is a useful tool to identify and monitor children at higher risk and aid them to access lead poisoning prevention services.
