Utah's Advancing Health Equity in Asthma Control through EXHALE Strategies - Asthma is an important public health issue in Utah. In 2022, about 11% of adults and 7% of children had current asthma1 which is approximately 271,157 adults and 67,351 children in Utah(BRFSS). Utah Department of Health and Human Services Asthma Program will use this funding to improve health outcomes for people with asthma through increased community partnerships and multi-component approaches to care based on the technical package EXHALE. Utah will address systems-level, environmental, and social drivers of disparities through strong partnerships that help implement at least 4 of the 6 EXHALE strategies in areas with a high asthma burden. Utah will work to strengthen our existing infrastructure (leadership, program management, strategic partnerships, surveillance, communication, and evaluation), and collaborate with diverse partners through a variety of organizations via the Asthma Task Force to implement the EXHALE strategies. Additionally, we will update our strategic plan to include equity and sustainability. Activities will advance our health equity partnerships with one community organization that serves and has been serving our focus population, local health departments, community health workers, and multi-sector agencies. We will also leverage evaluation findings to support health equity and sustainability and maintain and enhance our surveillance system.
