Georgia Asthma Control Program (GACP) - Georgia Department of Public health (DPH) seeks funding under CDC-RFA-EH-24-0016 to improve the reach, quality effectiveness, and sustainability of asthma control services across the state, and to reduce asthma morbidity, mortality, and disparities by implementing evidence-based EXHALE strategies. The state of Georgia, with a population of over 10.7 million (see Eligibility Documentation) ranks among the highest in asthma prevalence. Based on the number of emergency department visits, hospitalizations, and financial costs, the burden of asthma is significant (see Eligibility Documentation). The Georgia Asthma Control Program (GACP) within DPH has the capacity and expertise to make significant contributions to the work of the National Asthma Control Program) initiatives through implementing strategies in the EXHALE technical package. With strong support from DPH’s leadership, GACP proposes leveraging existing partnerships and forming new partnerships to build its existing infrastructure. For example, DPH’s Epidemiology Section will provide guidance on surveillance, data collection and dissemination. Members of the Georgia Asthma Advisory Board will provide input on program evaluation and effectiveness, as well as guidance in strategic planning. GACP proposes to implement the strategies in the EXHALE technical package through statewide partnerships with key stakeholders. GACP will provide training and technical assistance on implementing asthma self-management education (AS-ME) in schools, community-based organizations, and health systems. GACP will reduce smoking and exposure to smoke through collaborations with the Georgia Tobacco Use Prevention Program. Building upon projects currently underway, GACP will improve access to asthma self-management education and healthy homes assessments the home settings. To improve guidelines-based medical management, GACP proposes offering training and technical assistance on the CATAPULT framework. This framework guides health systems through quality improvement processes to make lasting impacts in chronic disease control. In addition, GACP proposes continued partnerships with The Children’s Care Network to support Linkage and Care Coordination and encourage partners and program participants to findhelp.org to receive information on AS-ME, smoking cessation resources, home visits, and other community resources. Finally, GACP will educate on environmental policies related to clean diesel technology and smokefree air through partnerships with the Department of Education, Public Health Districts, and the Environmental Protection Agency. As a program within DPH, GACP has the organizational capacity to implement the proposed strategies. DPH has both a Division of Financial Management and a Division of Grants Management which oversee federally funded programs. At the programmatic level, GACP has strong guidance from the Principal Investigator, who has over 20 years of public health experience. The Asthma Team Lead, who is responsible for the day-to-day activities of GACP, has 5 years of asthma-related experience with the program. With additional support from the Program Managers and Program Evaluator/Epidemiologist, the four members of the GACP form a strong team capable of implementing the strategies and activities of the EXHALE technical package.
