Advancing Health Equity in Asthma Control through EXHALE Strategies - The Kentucky Asthma Management Program in the Kentucky Department for Public Health has developed this proposal in collaboration with a group of partners considered key to success in designing, implementing and evaluating the work detailed in the project narrative. Utilizing data from hospitalization and emergency department visits, the Kentucky Behavioral Risk Factor Survey and Vital Statistics, we have identified our priority populations with asthma as children 0-17, African Americans, persons living in Appalachian counties, and persons with income less than $25,000. The proposed activities are designed to advance health equity and achieve the overall outcomes of better and coordinated care, lower health costs, and improved health in priority populations with reduced morbidity and mortality due to asthma. The project will expand capacity to deliver or refer to asthma self-management education (AS-ME) by training professionals and community health workers in evidence-based AS-ME including staff in schools, clinics and hospitals; improved systems to promote guidelines-based medical management, offer AS-ME and promote coordinated team care including bi-directional referrals through asthma quality improvement projects conducted with strategic partners; and decreasing the burden of indoor and outdoor environmental triggers for persons with asthma by increasing the understanding of healthy homes principles in people with asthma or caregivers of those with asthma, and increasing the number of schools and workplaces that have adopted environmental policies or best practices. We will avoid duplication of efforts by coordination and collaboration with our partners and stakeholders in the Kentucky Asthma Partnership and work together to improve strategic educational messaging and communication.
