Advancing Health Equity in Asthma Control through EXHALE Strategies - The Florida Asthma Program (FAP) of the Florida Department of Health’s (DOH) Bureau of Chronic Disease Prevention (Bureau) is seeking $2,900,000 through the funding opportunity Advancing Health Equity in Asthma Control through EXHALE Strategies (CDC-RFA-EH-24-0016) to collaborate with statewide partners for strengthening existing organizational infrastructure and implementing EXHALE strategies to prevent asthma-related emergency department (ED) visits and hospitalizations among residents with asthma. Florida is the third most populous state in the United States (U.S.) with approximately 20% of the population under 18 and about 13% living under 100% of poverty. The Bureau has been working for more than 25 years to improve health for all Floridians by addressing the burden of chronic diseases, among them asthma and its related triggers with a focus on priority populations including children and adolescents, racial/ethnic minorities, and low-income Floridians. For that purpose, the Bureau collaborates with a statewide network of public, non-profit, and private organizations including health systems, county health departments, Federally Qualified Health Centers, universities, health planning councils/coalitions, and community organizations. The FAP will work to enhance patient care coordination by linking patients, health care providers, and pharmacies to optimize education on asthma self-management and medication adherence. The FAP, also, looks forward to maintaining and expanding the home visiting program aiming at reducing asthma triggers to improve asthma control and reduce school/work absenteeism and ED and hospital inpatient visits. Furthermore, the FAP will work toward mitigating social support barriers by increasing multidisciplinary partnership and care within higher burden populations while maintaining its collaboration with the Florida Asthma Coalition to support school-based services and advocate for better environmental policies to reduce indoor and outdoor triggers. Also, collaboration will continue with Florida State University and other partners to maintain/improve infrastructure to guarantee the availability of resources necessary to maintain a robust surveillance and evaluation system to effectively inform decision-makers and stakeholders. Each year, the FAP will examine successes and barriers through the evaluation of implemented strategies and conduct quality improvement activities to ensure continued success throughout the duration of the cooperative agreement for the continued improvement of health and quality of life of Floridians living with asthma.
