The Minnesota Department of Health is requesting funds to implement strategies to address comprehensive asthma control in Minnesota. The MDH Asthma Program has extensive experience working with school, community and health care interventions and this funding will support a more coordinated approach that will expand the reach and accessibility of these needed services.
Key initiatives in this application include work with pharmacists to provide asthma self-management education, work with weatherization professionals to address linkages between housing and asthma, training licensed school nurses and public health nurses on guidelines-based asthma care and expansion of asthma home-based services to more counties across the state. We will also develop a new strategic plan for addressing asthma in Minnesota. Our work will bring together schools, health care, public health and community organizations to meet the needs of asthma in the community. We have built key strategic partnerships that will play key roles in achieving our objectives.
