Purpose: The AAFP has implemented the "Improving Adult Immunization Rates among Racial and Ethnic Minority Populations" project, which develops effective strategies, tools, and resources to increase and improve vaccination rates among racial and ethnic minority adults. Target Populations and Health Disparities: The AAFP will recruit up to 25 family medicine practices and residencies ins states and communities. Target Populations: Racial and ethnic minority adult patients 19 years of age and older. National Audienof Healthcare Providers: All active family physicians, residents, medical students, and other medical specialty organizations. Minority Community Partners: AAFP will collaborate with partners of minority organizations to increase vaccines among racial and ethnic minority adult populations. Phase 1 (Evaluation) Strategies and Activities: Strategy 1: Implementation of the AAFP's web-based Office of Champions Quality Improvement model - Activities: The recruited family medicine practices implemented the model in their practice to strengthen the team-based approach for increasing adult immunization rates among racial and ethnic minority patients and address the following national standards: immunization assessment; strong recommendation; administer to refer; and documentation. Outcomes: Increased vaccination rates among racial and ethnic minority adults; increased routing documentation of vaccination assessment in patient records; improved immunizations at the point of care, as measured through quality improvement; increased provider strong recommendations and increased documentation of adult immunization in EHR, and state/local IISs. Strategy 2: Development of comprehensive educational programs to teach effective evidence-based immunization strategies to close the gap of racial and ethnic disparities among adult patients - Activities: Educational opportunities are provided to the recruited family medicine practices and their teams to increase knowledge and awareness about the importance of immunizations, particularly among racial and ethnic minority adult patients. Outcomes: Improved use of provider strong recommendation; increased acceptance of vaccines among racial and ethnic minority adult patients; increased provider educational knowledge and awareness of strategies and interventions to improve vaccination within racial and ethnic minority populations; and improved collaboration and sharing of resources among immunization partners. Strategy 3: Broader adoption of resources and strategies to improve vaccination rates among racial and ethnic minority adult populations - Activities: Working with internal and external partners to develop resources and materials to address barriers and increase vaccination rates among racial and ethnic minority adult patients. Outcomes: Increased provider access to develop resources and strategies in rural and urban areas; increased number of healthcare providers utilizing educational resources and strategies; increased acceptance of vaccines among racial and ethnic minority adult patients, improved immunization referral procedures, where appropriate; and increased data exchanges among multiple healthcare providers who provide immunizations to adult patients. Phase 2 (Dissemination) Strategies and Activities - Strategy 1: Disseminate the "Improving Adult Immunization Rates Among Racial and Ethnic Minority Populations" project results and share best practices/successes - Activities: AAFP will promote results of the project through its communication channels (e.g., social media, website promotion, conferences, etc.). Outcomes: Dissemination and promotion of the project results; verification through the project results that the model contributed to improved adult immunization rates; and improved collaboration by sharing best practices/successes among immunization partners and minority community organizations.
