Purpose: Through its I Raise the Rates: Promotion of Influenza Immunization through Interprofessional Partnership (I Raise the Rates Project A) and I Raise the Rates: Promotion of Health Equity (I Raise the Rates Project B) projects, ACP proposes to implement nationwide, interprofessional quality improvement (QI) initiatives to increase adult and influenza vaccination coverage among high-risk patient populations and racial/ethnic minority adult patients. Project A will focus especially on the following aim as outlined in the NOFO: To bring about measurable improvements in influenza vaccination of adults with chronic medical conditions by expanding uptake of recognized immunization strategies among adult subspecialty providers. Outcomes: ACP has developed strategic approaches for Projects A that are designed to achieve the following outcomes. ACP has expanded upon the outcomes specified in the NOFO to reach key clinician audiences that are critical to achieving the aims of the I Raise the Rates Projects, bolstering overall progress toward the outcomes in the NOFO: • Increased engagement in adult influenza immunization efforts by internal medicine specialty, subspecialty care, and pharmacy providers • Increased provider understanding and awareness of recommendations and available resources to improve vaccination of high-risk adults • Increased uptake of developed materials and strategies by providers who care for high-risk adults • Increased number of specialty, subspecialty, and pharmacy providers who view immunization as part of their scope of practice • Increased number of specialty, subspecialty, and pharmacy providers who stock and offer vaccine In order to achieve these outcomes, ACP proposes to take an integrated, interprofessional team-based approach that builds from ACP’s decade of experience in implementing large-scale immunization programs including the I Raise the Rates QI program. Collaboration with national partner organizations will be a key component of our approach, and we have obtained commitments of partnership from the following organizations: American College of Cardiology, American Pharmacists Association, Endocrine Society, and the University of Chicago’s Chicago Center for Diabetes Translation Research. In collaboration with Project partners, we will: Develop interprofessional education resources; use these resources to establish a new section of ACP’s Adult Immunization Resource Hub dedicated to high-risk patients with chronic conditions; recruit subspecialists for the Program’s live and virtual education sessions; and significantly expand dissemination of materials and learnings via our partners’ communications channels. ACP will partner with the Johns Hopkins University Bloomberg School of Public Health to design and implement an evaluation approach that assesses the impact of Project.
