Service Provision of AIDS Resilience and Knowledge (SPARK) Project - Special Service for Groups (SSG) through its division Asian Pacific AIDS Intervention Team (SSG/APAIT), proposes to implement the Service Provision of AIDS Resilience and Knowledge (SPARK) project, a comprehensive high-impact HIV prevention services program that aims to prevent new HIV infections, improve HIV-related outcomes for people with HIV, decrease STI infections, and reduce HIV-related health inequities in Los Angeles County (LAC). SPARK will address the disproportionate outcomes of the HIV epidemic through the implementation and execution of core program pillars: diagnose, treat, prevent, and respond. SPARK will prevent new infections, increase access to care, reduce HIV-related disparities and promote health equity among Black/African American and Hispanic/Latinx young men who have sex with men (YMSM) ages 18 to 34 living with or at high risk for HIV in LAC. Diagnose Increased HIV testing among YMSM of color. By March 31, 2027, the program will have conducted 4,500 HIV tests of which 3,375 (75%) are YMSM of color. Increased HIV diagnoses among YMSM of color. By March 31, 2027, SSG/APAIT will identify 45 newly diagnosed HIV positive persons. Increased receipt of integrated screenings. By March 31, 2027, SSG/APAIT will provide 2,700 of YMSM of color with integrated screenings for STIs in conjunction with PS22-2203-funded HIV testing. Treat Increased linkage to HIV care among persons with newly diagnosed HIV. By March 31, 2027, SSG/APAIT will link 45 (100%) persons with newly diagnosed HIV to HIV medical care and ART initiation immediately Increased linkage to or re-engagement in HIV care among persons with previously diagnosed HIV, not in care. By March 31, 2027, SSG/APAIT will re-engage 18 (100%) persons previously diagnosed HIV, into HIV medical care. Increased access to Partner Services among persons diagnosed with HIV or STI. By March 31, 2027, 100% of persons newly and previously diagnosed with HIV and/or an STI will have received a referral to Partner Services. Increased access to appropriate medication adherence services. By March 31, 2027, SSG/APAIT will provide 100% of eligible clients with medication adherence services for HIV treatment or prevention. Prevent Increased screenings, referrals, and linkage to PrEP. SSG/APAIT will screen 100% and link 3,994 (90%) of HIV negative, YMSM of color to PrEP and/or nPEP services. Increased availability of condoms. SSG/APAIT will distribute 30,000 condoms to YMSM of color, regardless of HIV status. Increased access to prevention and essential support services. SSG/APAIT will provide 3,994 (90%) YMSM of color with at least one referral to a prevention and essential support service. Respond Increased collaboration and coordination with the LAC, Department of Public Health, Division of HIV and STD programs (DHSP) for cluster detection and response. By March 31, 2027, the program will reduce HIV transmission through participation in the health department’s surveillance data and HIV Molecular Cluster Detection in the region
