Project Title: Comprehensive High-Impact HIV Prevention Programs for Young Transgender Persons of Color Across Queens, Nassau, and Suffolk Counties

Long Island Crisis Center’s “Pride for Youth” program will address health disparities in New York’s Queens, Nassau, and Suffolk Counties. Young Transgender (YTG) persons, ages 20-29, account for 57.7% of newly-diagnosed transgender persons. Racial and ethnic minorities represent 90% of new infections. In addition, sexual contact transmission risk accounted for 86.5% of all new diagnoses. In 2019, 52 transgender people were newly diagnosed with HIV, and 26 deaths occurred among transgender people living with HIV/AIDS. Most notably, within Queens County, 24% of newly-diagnosed individuals also had a concurrent AIDS diagnosis. Furthermore, agency data demonstrates that 82% of YTG persons of color report having engaged in condomless penetrative sex with men who have sex with men.
LICC’s Pride for Youth program will address the health inequalities that fuel the HIV/AIDS epidemic amongst YTG persons of color. PFY will increase access to HIV rapid testing, integrated STD screenings, prompt linkage to life-saving care, medication adherence support, linkages to PrEP, nPEP, prevention interventions and other essential services, and outreach with condom distribution. Additionally, funded staff will collaborate with partner agencies and the health department, including assisting in cluster response and planning group efforts.
Continual data-driven methods and community member input will inform the development and execution of activities. By aligning the needs of YTG persons of color with the program’s success, the PS22-2203 program will prove to be integral to New York State’s efforts in addressing the HIV/AIDS health inequalities amongst YTG persons of color.
Over the five year project period, the PS22-2203 funded program will provide the following: conduct 3,000 HIV tests, identify 35 newly-diagnosed persons, provide 2,400 integrated STD screenings, conduct 480 rapid syphilis tests, deliver Personalized Cognitive Counseling to 750 clients, facilitate linkage to care within 30 days for both newly diagnosed and identified out-of-care clients, refer 100% of qualifying clients to Partner Services, deliver medication adherence support to 100% of qualifying clients, link 2,700 (90%) of clients to prevention or essential support services, link 846 individuals to clinical PrEP services, and distribute 60,000 condoms.
