Community Health Awareness Group (CHAG) proposes to provide high-impact HIV prevention services for Young Men of Color who have Sex with Men (YMSM of color; hereinafter referred to as YMSM) (13-34) and their partners regardless of age, gender, and race/ethnicity. CHAG’s project will align with the goals and objectives of Healthy People 2030 and the HIV National Strategic Plan (HIV Plan) (2021-2025) complementing the Ending the HIV Epidemic initiative to address the national HIV epidemic by reducing new infections, increasing access to care, and promoting health equity, in accordance with the CDC’s Ending the HIV Epidemic: A Plan for America Initiative and High-Impact HIV Prevention approach.
Combining evidence-based, cost-effective interventions targeting the right people in the right places, CHAG will work to 1) Prevent New HIV Infections; 2) Improve HIV-Related Health Outcomes of persons with HIV (PWH); 3) Reduce HIV-Related Disparities and Health Inequities; and 4) Achieve Integrated, Coordinated Efforts That Address the HIV Epidemic among all Partners and Stakeholders.
CHAG will utilize CDC PS 22-2203 funding to increase prioritized rapid HIV testing, status awareness and linkage to PrEP for YMSM at high risk of acquiring HIV in Detroit, referrals to Partner Services, woven together in a status neutral approach to include linkage to medical care and antiretrovirals (ARV) for prevention or treatment, essential services, including Life Points, CHAG’s Syringe Services Program (SSP), and re-engaging out of care clients with HIV treatment/prevention care. The program will include integrated STI/Viral Hepatitis screening and treatment, linking or referring status neutral clients to high impact behavioral interventions. Condoms will be distributed to all clients.
The program will include early intervention services to ensure that 100% of positive testers are linked with medical care through CHAG’s onsite clinic. This will increase receipt of HIV medical care and ART for those newly diagnosed with HIV, and those previously diagnosed but not-in-care. It will also include navigation to ensure that 100% of those testing negative are connected to PrEP and nPEP and condoms.
There will also be a safe space for the prioritized population at CHAG and through a collaboration with LGBT Detroit, an organization that serves the LGBTQ community. Within this safe space social determinants of health and syndemics affecting HIV-related outcomes impacting the priority population will be identified and addressed.
CHAG will work with the city and state health departments to respond to HIV outbreaks and clusters. The program will also incorporate program promotion, outreach and recruitment utilizing Social Network Strategies, targeted outreach and enhanced social media.
