APLA Health & Wellness (APLA H&W) - one of the nation’s oldest and most experienced grassroots HIV prevention and service agencies - requests funding through the CDC’s FY 2022 Comprehensive High-Impact HIV Prevention Programs for Young Men of Color Who Have Sex with Men and Young Transgender Persons of Color (PS22-2203) to implement El Saludable!, an innovative, comprehensive HIV prevention initiative designed to significantly reduce the number and incidence of new HIV infections in Los Angeles (LA) County, California. El Saludable! is a Spanish language term whose literal meaning is something akin to “the healthy ones” but which in everyday usage reflects an overall sense of health, well-being, confidence, and empowerment. The El Saludable! program will work to prevent HIV among young Latinx men who have sex with men (YLMSM) ages 18 - 34. El Saludable! will implement an integrated matrix of high-impact HIV prevention activities working out of three neighborhood-based APLA H&W locations situated in the hardest-hit areas of LA County: the Gleicher / Chen Health Center in the Baldwin Hills region; the agency’s Long Beach Health Center; and APLA H&W’s newest Health Center on the campus of the Martin Luther King / Charles R. Drew Medical Center in South Los Angeles. El Saludable! will apply state-of-the-art findings and emerging strategies to connect with and engage the highest-risk young Latinx MSM (YLMSM)in LA County to prevent new HIV infections, connect HIV-positive and HIV-negative YLMSM to culturally competent, high quality HIV, PrEP, and PEP medical services, and build a greater sense of community, social connectedness, and pride among local YLMSM.The overarching purpose of the proposed program is to reduce and prevent new cases of HIV transmission among young Latinx MSM (YLMSM) and their partners in Los Angeles County, California in accordance with both the HIV National Strategic Plan and the CDC’s High-Impact, Status-Neutral HIV Prevention approach. The project will accomplish this goal through a linked, integrated program that: a) recruits YLMSM from five targeted LA County Health Districts that are underserved in regard to HIV education, testing, and services to serve as community leaders and organizers using the Community PROMISE intervention; b) increases utilization of both stand-alone and integrated HIV/STI counseling and testing services by high-risk YLMSM populations; c) effectively links both newly identified and previously identified YLMSM with HIV to Partner Services and to high-quality, in-house medical care, including same-day ART; d) refers and links HIV-negative YLMSM at high risk for HIV to high-quality, in-house PrEP and PEP services, including same-day PrEP starter packs; e) provides ongoing treatment retention and medication adherence services to support long-term HIV and PrEP treatment engagement; f) provides comprehensive access to condoms and safe sex kits; g) increases outreach to and engagement with high-risk young Latinx MSM in Los Angeles County, including through field and home-based testing; h) maintains a project-specific safe space that addresses at least two key social determinants of health; and i) offers ongoing HIV health education, risk reduction, and linkage to essential prevention and supportive services. The program will place a particular emphasis on reducing new HIV infections by addressing both health disparities and additional underlying issues such as stigma, lack of access to tailored health education and health services, and lack of supportive social networks among YLMSM.
