AltaMed is proposing a high-impact HIV prevention program targeting Category A: Young MSM of Color and their partners in Los Angeles County with a particular focus on Latinx MSM who are experiencing the highest local disease burden. AltaMed is expanding its CDC-PS21-2102 high-impact prevention efforts which explicitly targets cultural, subcultural and affinity groups within the Latinx MSM community: (Indigenous people of Oaxaca, Salvadoran immigrant community, Queer, punk rock culture, and Gay Vaquero community. Proposed PS22-2203 activities will leverage this strategy of targeting Latinx MSM subcommunities and expand outreach and engagement to four subpopulations among young, Latinx MSM 13-29 years of age: (1) Bear/ Cub YMSM; (2) Tea Party Community; (3) Drag Community; and (4) Unhoused/ unstably housed YMSM. Each year, AltaMed will test 500 Latinx YMSM and identify 8 new positives in LA County Service Planning Areas (SPA) 6: South, SPA 7: East, and SPA 4: Metro (specifically, within the Central Health District in SPA 4). AltaMed will engage clients in an adapted version of the evidence-based Sin Buscar Excusas (No Excuses) risk-reduction intervention that will be tailored for each Latinx YMSM subpopulation. AltaMed also will execute a diffusion strategy similar to the Popular Opinion Leader model of HIV prevention intervention by identifying and assembling a coalition of gatekeepers and influencers in the community, The Coalition for Underground Nightlife, and in social media within each subpopulation who will be trained to serve as “HIV Influencers”. Targeted outreach at formal and informal subculture events, venues, and gathering spaces (physical and virtual) will link clients to proposed services: HIV counseling and testing, in-home testing, integrated STI/Hep C testing, linkage-to-HIV care, and prevention and essential support services to optimize health outcomes. High-risk, negative persons will be offered STD screening and treatment, pre- and post-exposure prophylaxis (PrEP/PEP), and risk-reduction supports. The program will leverage AltaMed’s enterprise-wide integration of HIV prevention with its primary care and pharmacy services, as well as enhance prevention efforts supported by CDC, CA Office of AIDS, and LADPH Division of HIV and STD Programs. By the end of the five-year project period, AltaMed will achieve the following outcomes: • Conduct at least 2500 HIV tests among high-risk Latinx MSM (75% YMSM of color); • Identify at least 36 unaware HIV-positive Latinx YMSM; • Refer at least 90% of clients to PESS; 400 clients will participate in Sin Buscar Excusas; • Pilot HCV rapid testing integration into in-person HIV testing events; • Offer condoms to 100% of clients; 76,000 distributed; • 90% of high-risk negative Latinx MSM will be referred to PrEP or PEP; • Link 90% of newly-diagnosed HIV-positive MSM to HIV medical care =30 days; • Re-engage 100% of previously diagnosed with HIV infection into care within =30 days; • Refer 100% of HIV or STI-positive Latinx MSM to Partner Services; • Provide medication adherence services to 100% of those needing treatment support
