Project Title: Comprehensive High-Impact HIV Prevention for Young Men of Color Who Have Sex With Men and Their Partners Organization Name: Wyckoff Heights Medical Center, Department of Internal Medicine Address: 374 Stockholm Street, Brooklyn, NY 11237 Project Director/Principal Investigator: To be Hired Grant Contact: Anca Giurgiulescu, Director, Grants and Evaluation Telephone: 929-432-3486 Email: AGiurgiulescu@wyckoffhospital.org Congressional District: NY-7; Website: www.wyckoffhospital.org Total Budget Requested (5 years): $2,493,435.00 Overview: The Wyckoff CDC HIV Prevention program will contribute to the national strategy to reduce HIV infections by 75% in the next five years by providing comprehensive, high-impact HIV prevention services tailored and targeted to young men of color who have sex with men (YMSM of color) aged 13-34 in the Brooklyn and Queens communities served by our non-profit community hospital. The program aims to provide 400 HIV tests and identify 8 new HIV diagnoses annually and link a minimum of 300 YMSM of color to comprehensive HIV prevention services such as integrated HIV/STI/HCV testing, PrEP, and PEP. The program will provide early linkage to HIV care for all clients who test HIV positive at the Wyckoff Positive Health clinic no later than 30 days after HIV diagnosis, and provide a range of supportive services including mental health and substance use counseling, medication adherence counseling and case management, and individual and group-based behavioral health interventions to support YMSM of color in staying engaged in HIV prevention and treatment services. Outcomes: 1. Diagnose • Wyckoff will conduct 400 HIV tests annually, and will identify 8 new HIV diagnoses and 8 previously diagnosed, out of care individuals annually. • Wyckoff will connect a minimum of 300 (75%) of all clients to integrated HIV/STI/HCV screenings. 2. Treat • Wyckoff will link to HIV medical care and ART, ideally within 3 days and no later than 30 days from diagnosis a minimum of 7 (90%) of new HIV diagnoses and 7 (90%) of previously diagnosed, out-of-care YMSM of color annually. • All (100%) of the newly diagnosed and previously diagnosed clients identified through testing will be referred to partner services offered by the NYC Department of Health. 3. Prevent • Wyckoff will educate and refer to in-house PrEP and PEP services all (100%) eligible YMSM of color who are HIV negative and at high risk for HIV infection or ongoing exposure, with a goal of linking 300 YMSM of color annually to PrEP/PEP services. • Wyckoff will provide 10 condoms per safe sex pack per client and distribute a minimum of 4,000 condoms annually. 4. Respond • Wyckoff will engage regularly with the NYC and NYS Health Departments through ongoing activities such as trainings, quality improvement, surveillance and partner services, and “Dear Provider” communications for cluster detection and response and will collaborate with community partners to more effectively engage Latino MSM clients.
