The Southwest Center for HIV/AIDS, Inc. (SWHIV) will implement a High Impact HIV Prevention Project focused on reducing new HIV infections, increasing access to and retention in HIV medical care, and promoting health equity. Through implementation of Choosing Life: Empowerment! Action! Results! (CLEAR) intervention, the Mpowerment project, and integrated HIV/STI testing and access to care, the project will strengthen the services network to reduce new HIV infections, increase access to care for those living with HIV, and promote health equity for persons living with, and at increased risk for, HIV infection. SWHIV will subcontract with 1n10, Inc. to reach its target population. The target population includes young Hispanic MSM between the ages of 13-29 who are living with HIV or at high-risk for HIV infection. The project's secondary target population is African American MSM ages 13-29. The purpose of the project is to:1) Increase access and accessibility to HIV and STD testing among those at highest risk for HIV infection, link newly diagnosed HIV-positive individuals to HIV medical care and critical support services, and re-engage previously diagnosed out-of-care HIV-positive persons in care;2) promote retention of HIV-positive persons in HIV medical care and promote use of antiretroviral therapy;3) ensure wide availability and accessibility of condoms for HIV-positive and high-risk negative (HRN) persons; and4) Ensure that HRN persons have access to critical prevention and essential support services, including PrEP and/or nPEP.The project is built on the foundation of evidence-based/informed strategies. Project activities will be monitored and adjusted to ensure that individuals are being served in an appropriate manner and that project outcomes are met. Project outcomes include:1) Reduce HIV transmission from people living with HIV, and reduce HIV incidence among young Hispanic MSM ages 13-29;2) Increase HIV-positive and High-Risk Negat ive persons' access to care and increase the number of HIV-positive persons with suppressed viral loads;3) Increase the number of partners of HIV-positive persons who are aware of their risk for HIV and are tested for HIV;4) Increase the number of high-risk negative MSM who receive and use PrEP and/or nPEP; and5) Increase the number of HIV positive persons and HRN persons who use condoms consistently and correctly.
