This is a Category A HIV prevention services for Young Men of Color who Have Sex with Men (YMSM) and their partners regardless of age, gender, and race/ethnicity proposal on behalf of the Long Island Association for AIDS Care, Inc. (LIAAC) in response to Funding Opportunity CDC-RFA-PS17-1704.Our primary target population is Young (ages 16-29) MSM of color. According to the 2014 DOH Annual HIV/AIDS Surveillance Report, Long Island (Nassau and Suffolk Counties), excluding NYC, had the highest number of newly diagnosed HIV cases in among minority males with MSM transmission in the NYS Ryan White Region. Racial and ethnic minorities have a documented disproportionate burden of HIV/AIDS infection in the Nassau/Suffolk Metropolitan Statistical Area (MSA). LIAAC is proposing a YMSM Alternative Prevention and Care (YMSM APAC) program that will address gaps in HIV prevention for these individuals who are at highest risk of acquiring HIV. Through targeted testing, diligent program promotion, outreach and recruitment, and formalized collaborations, LIAAC will deliver comprehensive HIV prevention and assist in reducing morbidity, mortality and related health disparities. Individuals who are newly or previously identified as HIV-positive will be enrolled into the program?s High Impact Prevention (HIP) behavioral intervention CLEAR, linked to HIV medical care, provided medication adherence services, and receive assistance with Partner services. High-risk HIV-negative (HRN) individuals will be referred for PrEP/nPEP services, enrolled into the program?s HIP behavioral intervention VOICES/VOCES and/or referred to Mpowerment. PLWH/A and HRN persons will be enrolled into the Linkage to Care program and be provided with assistance for essential and supportive services. The program will offer condoms to all individuals free of charge as well as provide education on condom use. Our program will include HIV, HCV and STI (Gonorrhea, Chlamydia, and Syphilis) testing, t argeted to YMSM of color, who live or congregate in disenfranchised Long Island communities with high prevalence of HIV/AIDS. Utilizing a specially equipped van, Care Navigators will conduct street as well as social media outreach to identify and engage the target population. Annually, we will: (1) encounter 1,000 individuals; (2) conduct 225 HIV tests; (3) identify 7 newly diagnosed HIV-positive persons and link them to HIV medical care, provide medication adherence services, refer them to Partner Services, and refer them to HIP behavioral interventions (Healthy Relationships, Mpowerment); (4) re-engage 12-13 PLWH/A into HIV medical care; (5) enroll 20 PLWH/A into CLEAR; (6) 225 STI tests; (7) 200 HCV tests; (8) enroll 40 HRN into Linkage to Care program and referred HRN to PrEP/nPEP services; (9) 24 individuals will be enrolled into the program?s HIP behavioral intervention VOICES/VOCES; (10) provide 10 couples with Couples HIV Testing and Counseling (CHTC); (11) provide 10 individuals with Personalize Cognitive Counseling (PCC); (12) refer HRN to HIP behavioral interventions (Mpowerment); (13) distribute 40,000 condoms; (14) conduct quarterly Youth Advisory Board meetings; (15) enroll 10 individuals into the Mentoring Program.
