HIV prevention services for Young Transgender Persons of Color and their partners regardless of age, gender, and race/ethnicity. - AIDS Project Los Angeles Health and Wellness (APLA H&W), a multi-service Federally Qualified Health Center, in partnership with The LGBTQ Center of Long Beach (The Center), proposes to implement ?Trans Connections,? a comprehensive high-impact HIV prevention project for Young Transgender Persons of Color (YTG) and their partners. Trans Connections specifically targets young (18-29) transgender women of color who reside in the South Los Angeles, and South Bay Service Planning Areas (SPAs) of Los Angeles County. Identifying undiagnosed HIV positive transgender individuals, linking them to quality ongoing medical care, ensuring their adherence to medication and suppressing their viral load are essential actions to achieving the goals of the National HIV/AIDS Strategy Updated to 2020 and CDC?s High-Impact HIV Prevention approach: reducing new infections, increasing access to care, and promoting health equity. Increasing knowledge, awareness, and access to Pre-Exposure Prophylaxis (PrEP) and Post-Exposure Prophylaxis (PEP) among high-risk HIV-negative transgender individuals are also essential steps to achieving these goals. APLA H&W will establish and/or build upon relationships with organizations serving YTG person of color to ensure seamless service delivery. Specifically, we propose to offer the following services:Comprehensive program promotion, outreach and recruitment.HIV testing including Couples HIV Counseling and Testing and integrated screening for STDs, hepatitis and TB.Linkage/navigation services for HIV-positive individuals, including the provision of CLEAR behavioral intervention and Peer Support and EVERY DOSE EVERY DAY (E2D2) medication adherence interventions. Navigation services for HIV-negative individuals, including the provision of PrEP, and the PROMISE behavioral intervention. Condom distributionOver the course of the five-year program, APLA H&W will achieve the following outcomes: 1. Increase the number of YTG POC who are tested for HIV by at least 700.2. Increase the number of YTG POC who are diagnosed with HIV infection who were previously unaware of their infection by at least 27.3. Increase the number of newly diagnosed HIV+ YTG POC who are linked to HIV medical care within 30 days by at least 44. 4. Increase the number of previously diagnosed, out-of-care, HIV+ YTG POC who are linked in HIV medical care within 30 days of recent HIV test by at least 33.5. Increase the number of HIV+ YTG POC who are referred to or receive medication adherence services by 84.6. Increase the number of HIV-positive YTG POC who are offered Partner Services, by 49. 7. Increase the number of HIV-positive YTG POC who are provided or referred to CLEAR or another HIP behavioral intervention by 84. 8. Increase the number of HRN YTG POC who are referred to PrEP and/or n-PEP by 605. 9. Increase the number of HRN YTG POC who receive PrEP by 453.10. Increase the number of YTG POC who are offered condoms by at least 1,700.
