Health Care Center for the Homeless, Inc. (HCCH), dba Orange Blossom Family Health, provides medical, dental and behavioral health services in Orange, Osceola and Seminole Counties in Central Florida for homeless and low-income housed but uninsured and under-insured residents. HCCH will conduct high-impact HIV prevention services in the organization's tri-county service area, addressing the four pillars of HIV prevention: Diagnose, Treat, Prevent and Respond. The population of focus will be high-risk Black heterosexual men and women ages 13 through 60 plus. Diagnose will occur through providing HIV testing of at least 600 persona annually within the identified population of focus along with offering integrated screenings through targeted outreach and community events. Treat will take place through the referral of persons newly diagnosed with HIV and those previously diagnosed with HIV not in care into medical care and essential support services. Prevention will be accomplished by referring HIV negative persons to Pre-Exposure Prophylaxis (PrEP) and essential support services and through condom distribution. Respond will take place as HCCH partners with local health departments to help address HIV clusters within the community. Prevention activities will also take the form of education and awareness within the HCCH tri-county service area through social media campaigns and community engagement.
