As one of the largest HIV programs in Louisiana, and poised to make a greater impact, the HIV/AIDS Alliance for Region Two, Inc. dba Open Health Care Clinic (OHCC) will enhance existing organizational capacities for the promotion, delivery and sustainability of HIV prevention programs and treatment services. The proposed strategies align with the Ending the HIV Epidemic Initiative by diagnosing HIV as soon as possible through RAPID testing and providing rapid access to treatment. As the largest Pre-Exposure Prophylaxis (PrEP) provider in East Baton Rouge Parish, the Agency is also instrumental in the prevention of new HIV cases. Short-term Outcomes • Increase persons who are aware of their HIV status o By June 30, 2022, increase from 4,200 to 7,200 (58%) HIV tests with a minimum (5,400) of those tested belonging to the target population of men having sex with men (MSM) and heterosexual African American men and women between the ages of 18-45 o By June 30, 2022, increase from 90 to 150 (60%) individuals aware of their new HIV diagnosis • Increase receipt of integrated screenings o By June 30, 2022, provide screening for syphilis, gonorrhea, chlamydia and hepatitis C at one large scale testing event per year o By June 30, 2022, engage 1,000 high-risk negative repeat testers in Integrated Screening by offering opt-out Sexually Transmitted Disease screening during every visit • Increased receipt of HIV medical care and antiretroviral therapy (ART) among persons with newly diagnosed HIV o By June 30, 2022, maintain at least 100% linkage rate for all newly diagnosed (150) PLWH persons linked to HIV medical care and ART within 30 days of diagnosis • Increased receipt of HIV medical care and ART among persons with previously diagnosed HIV or not-in-care o By June 30, 2022, increase from 28 to 54 individuals previously diagnosed with HIV that are re-engaged into care • Increased access to partner services o By June 30, 2022, refer 100% of persons newly diagnosed with HIV to Partner Services o By June 30, 2022, 100% of HAART Clinic, Primary Care Department and the Department of Social Services will receive training on the importance of Partner Services o By June 30, 2022, refer 100% of persons previously diagnosed with HIV to Partner Services • Increased access to PrEP o By June 30, 2022, educate 100% of persons at high-risk for acquiring HIV to PrEP o By June 30, 2022, increase patients referred to PrEP from 180 to 300 • Increased availability of condoms o By September 30, 2021, establish 20 condom distribution sites o By June 30, 2022, maintain 100% condom offerings to persons diagnosed with HIV and persons at the greatest risk for HIV
