The California Prevention Training Center (CA PTC), through the fiscal sponsorship of the University of California at San Francisco, submits the following application for Components A (Regional Center) and Subcomponents A1 (HIV preventive services in STD clinics) and A2 (STD Fellowship) in response to CDC’s National Network of Prevention Training Centers (NNPTC) Notice of Funding Opportunity PS20-2004. Component A Regional Center: CA PTC proposes a high-quality clinical training program for clinicians and health care organizations (HCOs) with a goal of improving knowledge, skills and practices for clinical STD prevention and care. Working collaboratively with the other NNPTC National Centers, we will assess STD clinical training needs and service gaps, promote training and resources to effectively reach clinicians providing STD care. We will participate in NNPTC activities with regional and national partners, evaluate our trainings and develop effective strategies to increase the quality of STD training and technical assistance (TA). We will continue to strong working relationships with partners working in private HCOs such as Kaiser Permanente, as STDs continue to be diagnosed outside of STD specialty clinics. We will adapt our training/TA to meet the needs of busy providers, including utilization of distance learning and online learning communities (LC). Short term outcomes include: 1) improved access to and reach of T/TA for priority providers, 2) increased partnerships between CA PTC and its stakeholders, 3) LCs that improve provider knowledge/skills to provide STD clinical preventive services, 4) continued participation in the STD Clinical Consultation Network (CCN), and 5) increased provider use of the NNTPC newsletter. Intermediate-term outcomes include increased capacity and provision of culturally informed STD clinical preventive services by priority clinical settings serving key populations. Subcomponent A1: CA PTC will identify gaps by assessing the state of HIV prevention services in STD specialty clinics in our designated region. Based on our assessment we will develop and deliver training/TA to implement or scale up HIV prevention services, including PrEP and nPEP. In order to provide ongoing support for clinics as in this process, we will conduct site visits to analyze workflows and protocols onsite in real time. We will host reverse site visits at our model clinic in San Francisco so that our partners may observe procedures for PrEP/nPEP implementation. Finally, we will conduct LCs for STD specialty clinic providers/leaders to address ongoing implementation issues. Short term outcomes include: 1) improved ability of STD clinics to identify those who could benefit from PrEP/nPEP, 2)increased identification of new HIV infections in STD clinics, 3) increased identification of virally unsuppressed people in STD clinics, and 4) increased number of individuals in STD clinics who are offered HIV PrEP/nPEP. Intermediate outcomes include increasing capacity of STD clinics to provide HIV PrEP/nPEP and linkage to care; and increased STD clinics who provide PrEP. Subcomponent A2: Based on our two decades of experience, we propose to continue training STD Clinical Prevention Fellows in partnership with the California Department of Public Health and San Francisco Department of Public Health (SF City Clinic). Our comprehensive program encompasses training in research, program evaluation, public speaking/education, and clinical STD care. Short term outcomes include achievement of the fellowship’s core competencies in each domain of training. Intermediate outcomes include development of skills needed for a future leadership role in public health or academia in the field of STD/HIV.
