Abstract The Sylvie Ratelle STD/HIV Prevention Training Center (Ratelle PTC), a program of the Massachusetts Department of Public Health, proposes to continue its efforts as a Regional PTC for the New England states and Florida. Ratelle PTC proposes the following: 1. Component A: Serve as a Core Regional PTC to address New England and Florida’s STD prevention and treatment training, technical assistance, (T/TA) and systems change needs. We will determine the needs of the coverage area and prioritize key providers/clinical settings for training and technical assistance (T/TA). We will develop, disseminate, evaluate, and improve T/TA to advance STD care by individual health care clinicians and groups of providers as well as health care systems in our coverage region, integrating quality improvement approaches and principles in healthcare settings. Training delivery methods will be a mix of didactic, experiential, and distance learning utilizing webinars and the ECHO model of training (Extension for Community Healthcare Outcomes, www.echo.unm.edu). Training content will be based on current CDC STD Treatment Guidelines and recommendations for providing quality STD clinical services in primary and specialty care settings. Our efforts will be guided by partnerships with our regional CDC-funded STD PCHD recipients (Strengthening STD Prevention and Control for Health Departments), other national training centers, academic institutions, and others. We will develop strategies which specifically target T/TA and systems change approaches for clinicians and organizations that clearly demonstrate service to adolescents and young adults, pregnant women, men-who-have-sex-with-men (MSM), transgender women in high-morbidity settings, and racial and ethnic minorities disproportionately affected by STDs. 2. Subcomponent A1: Serve as the regional STD Clinical Prevention Training Center (STD PTC) to address New England and Florida’s HIV preventive services’ training, technical assistance, (T/TA) and systems change needs and support the scale-up of PrEP/nPEP in STD clinics. Our efforts will specifically focus on the seven Florida counties and one Massachusetts county that have been identified as geographic areas with target populations hardest hit by HIV in CDC’s Ending the HIV Epidemic: A Plan for America: First Year Geographic Focus. We will conduct in-person assessments of STD clinics, to document STD and HIV preventive services that are currently provided, identify gaps to expand capacity and scale up services, and assess quality of services provided to inform T/TA for these priority settings. We will develop and deliver T/TA, and establish learning communities of clinic medical directors and key stakeholders. Our efforts will strongly focus on building the capacity of Florida providers to prescribe and follow patients in the statewide Florida PrEP Telehealth-On-Demand program. 3. Subcomponent A2: Establish a STD Clinical Prevention Fellowship in the coverage region. We will develop and implement a systematic process to recruit, select, and train fellows from the coverage area, providing an opportunity for clinical, research, and public health mentorship for STD-focused career development. These activities support the long-term outcomes of this Cooperative Agreement: (1) increased availability and accessibility of a trained, skilled and competent clinical workforce to prevent and control STD/HIV, (2) increased fellows who remain in STD/HIV prevention academic and/or public health careers, (3) decreased transmission of STD/HIV in the U.S. (4) decreased incidence of antibiotic-resistant GC, (5) decreased morbidity and mortality from STD/HIV.
