Our proposed intervention, Community Health Interventions to Advance Self-Management of Asthma (CHI-ASMA), aims to improve health outcomes for communities disproportionally impacted by the burden of asthma due to social determinants of health. We plan to develop and implement a coordinated series of community health initiatives, targeting at risk populations in an effort to educate and empower patients and families to improve asthma control. Specifically, we will leverage our online asthma patient community, digital media, and strategic partnerships to deploy evidence-based and health literate asthma management resources, incorporating EXHALE and CCARE messaging and strategies. We have identified the following short-term outcomes for the proposed educational intervention that we will achieve during the project period. Promote Updated Recommendations in EPR-4 to Clinicians and Patients/Caregivers 1. Increase awareness of updated guidelines for asthma management. 2. Improve patient/caregiver knowledge of asthma control and use of asthma action plans. 3. Develop partnerships with state health departments and encourage them to incorporate the ASTHMA Care curriculum as part of their asthma education programs. Promote Guidelines-Based ASTHMA Care for Military Service Members 1. Form a multi-disciplinary clinical team. 2. Train clinical staff to deliver guidelines-based care and education. 3. Train chronic disease managers and educators in guidelines-based education. 4. Improve self-management of workplace asthma by military service members with asthma. Plan and launch three national awareness and education programs – (1) respiratory illnesses; (2) inhaler usage and asthma control; and (3) childhood allergic asthma. 1. Developed and launch three national awareness and education campaigns. 2. Increase knowledge about illness prevention strategies. 3. Increase the number of community members who get vaccinated. 4. Increase the number of community members that practice good health habits. 5. Increase number of community members who report having an asthma action plan they use to guide self-management and manage asthma episodes. 6. Increase knowledge and awareness about different types of inhalers and how to use them. 7. Increase awareness about pediatric allergic asthma. Partner with Community Health Workers (CHWs) and empower patients/caregivers to assess homes for asthma triggers 1. Develop partnerships with CHWs. 2. Increase knowledge about asthma triggers and abatement strategies. 3. Develop educational resources CHWs can use during in-home and virtual interventions. Promote Evidence-Based Asthma Care for Pregnant and Lactating Women 1. Increase awareness about the challenges in managing asthma for pregnant and lactating women.
