Florida Colorectal Cancer Control Program - Colorectal cancer (CRC) remains a leading cause of cancer-related deaths in the United States, with Florida carrying a significant disease burden. Despite the proven benefits of early detection and prevention through screening, variances in access persist among sub-populations in Florida. The Florida Department of Health (FDOH), through its Colorectal Cancer Control Program (CRCCP), is committed to closing these gaps by implementing evidence-based interventions (EBIs) to increase screening rates statewide. FDOH will continue implementing the Florida CRCCP, building on sixteen years of successful program execution. Services will focus on uninsured individuals over 45 years old, at average risk of CRC, and living at or below 200% of the federal poverty level. The program aims to expand its reach annually, serving thousands of high-need individuals over the grant cycle. FDOH will collaborate with clinical partners to implement evidence-based interventions such as strengthening patient reminders, provider reminders, and electronic health record prompts, while prioritizing historically underserved communities through robust local partnerships, tailored outreach materials, and our county-level public health network. To further reduce barriers, FDOH will leverage partnerships with Florida’s four National Cancer Institute designated cancer centers to connect patients to follow up colonoscopies and supportive services such as childcare and transportation, and ensure high-risk patients receive timely colonoscopies in accordance with clinical guidelines. FDOH will also enhance data collection and reporting through the Florida Cancer Data System to track screening rates, measure outcomes, and refine our strategies. Finally, we will foster sustainable practices by integrating these interventions into existing clinical workflows to support long-term improvements in cancer screening among all clinic populations. The CRCCP’s five-year strategy prioritizes sustainability by cultivating CRC screening champions within health systems, integrating EBIs into electronic health records, and providing free stool-based screening tests to eligible patients. Partnerships with Federally Qualified Health Centers and community health centers will further address screening gaps, ensuring improved access to CRC screening for the target population. Through continuous monitoring, technical assistance, and targeted outreach, the program will enhance patient adherence, provider engagement, and system-wide screening integration. By strengthening partnerships, expanding access to preventive services, and addressing healthcare disparities, the CRCCP will contribute to the national goal of achieving 80% screening in every community. Through strategic interventions and rigorous evaluation, the program aims to reduce CRC incidence and mortality, improve early detection, and enhance health outcomes for Florida’s most vulnerable populations.
