Public health and health system strategies to increase rates of Colorectal Cancer Screening in Vermont. - Vermont Project Abstract Summary The Vermont Colorectal Cancer (CRC) Screening Program aims to increase CRC screening rates among adults aged 45 to 75, particularly those with lower screening prevalence, such as individuals served by Federally Qualified Health Centers (FQHCs) and community health centers. The program will collaborate with three FQHCs and one community health center, all serving populations with historically lower CRC screening rates. In Vermont, adults with low income and those in rural areas are less likely to undergo CRC screening compared to other groups. To address these disparities, the Vermont CRC Program will support partner clinics in implementing evidence-based interventions (EBIs) designed to boost CRC screening rates. Additionally, we will collaborate with a state quality improvement organization to provide technical assistance, quality improvement initiatives, training, and evaluation support. By the end of the performance period, the Vermont CRC Program aims to achieve the following key outcomes: 1. Enhanced/Increased Implementation of EBIs: Each partner clinic will implement at least three multicomponent EBIs to improve screening processes. 2. Increased Return of Stool Test Kits and Completion of Colonoscopies: We anticipate at least a 5% increase from baseline in these metrics during the performance period. 3. Improved CRC Screening Rates: We expect at least a 10% increase in screening rates for clinics with baseline measures below 50%, and at least a 5% increase for those with baseline measures at 50% or higher. Achieving these short-term outcomes will contribute to preventing CRC and increasing early-stage diagnoses, where treatment options and outcomes are more favorable. In the long term, the program aims to reduce CRC incidence and mortality, thereby addressing health disparities among underserved and high-risk populations. Vermont is committed to leveraging its expertise in evidence-based interventions to enhance screening rates through this cooperative agreement.
