A dispersed population, limited healthcare infrastructure, diverse cultural and linguistic backgrounds, and vulnerability to natural disasters exacerbate these challenges. This project aims to implement evidence-based strategies to prevent and control chronic diseases in FSM, focusing on reducing tobacco use, managing diabetes, and improving overall public health through community outreach, education campaigns, and enhanced healthcare services. Target Population: The primary target population includes underserved communities within FSM, particularly those at higher risk for chronic diseases.
