A Strategic Approach to Advancing Health Equity for Priority Populations with or at Risk for Diabetes - Activating a Community of Practice through the YMCA Network and Multisectoral Partners to Reach Priority Populations and Scale and Sustain the National DPP – COMPONENT C Over a five-year period, YMCA of the USA (Y-USA) will convene, administer, support, and participate in a multisectoral Community of Practice and Learning Hub that will explore and test multiple models across the country to increase access and engagement of priority populations in the National Diabetes Prevention Program (DPP). Priority populations include people living with prediabetes who are 65 years and older, Hispanic, Latino, and Black individuals, immigrant populations, refugee populations, and people living at or below the federal poverty level. PURPOSE: To increase access to and retention in the National DPP, Y-USA will leverage the robust experience of its nationwide network of delivery organizations and multisectoral partners to convene an interdisciplinary Community of Practice composed of local Ys, YMCA State Alliances, state health departments, CBOs, health care organizations, payers, as well as trusted individuals and organizations across the country. This multidisciplinary learning and action network will act as a Learning Hub, engaging four Planning Collaboratives each with a focus on one of the Component C strategies (Administrative Infrastructure, Participant Referral Processes, Program Delivery and Participant Support, and Payment/Coverage). Each Planning Collaborative will be tasked to provide thought leadership, subject matter expertise and guidance, and drive action to address the goals. OUTCOMES: Collectively, the Community of Practice and Learning Hub will explore and test multiple network models and approaches across the nation that increase multisectoral partnership networks to reach priority populations, and successfully identify, engage, and enroll 10,000 new adults into the National DPP by the end of the five-year project period.
