Through Racial and Ethnic Approaches to Community Health for Equity (REACH for Equity), Beaumont Health (soon to be Corewell Health East [CHE]) will serve residents of Wayne County, Michigan (MI), home of the City of Detroit, with a total population of 1,774,816. With guidance and support from an active, cross-sector network of community partners, REACH for Equity will prioritize individuals and families of color— African-American/Afro-Caribbean and Latinx descent, residing in Wayne County, MI—with a special focus on those living in poverty. REACH for Equity will address key health disparities related to Nutrition/Healthy Food Access, Physical Activity, and Continuity of Care in Breast-feeding (Component A), through a diverse menu of evidence-based interventions, including Health Communications Campaigns (leveraging traditional and social media); efforts to scale-up proven EBPs with partners across the community; and engaging in collaborative strategies with a cross-sector Partner Network to promote systems-level/policy solutions to health disparities related to nutrition, physical activity, and continuity of care in breastfeeding.
