A Social Determinants of Health Accelerator Plan in Food/Nutrition Security and Built Environment for the Greater Trenton Area in New Jersey - Trenton is New Jersey’s capital city, one of the state’s poorest cities situated within Mercer County, one of the state’s wealthiest counties. Decades of disinvestment have resulted in inequitable health and socioeconomic outcomes between Trenton and the rest of the county and state overall. Trentonians face disproportionate rates of chronic disease, exacerbated by food insecurity and housing instability. Trenton Health Team (THT) and our partners, including the City of Trenton, are working to change that. THT is pursuing this Social Determinants of Health (SDoH) Accelerator Plan opportunity at this time, with a focus on the CDC priority SDoH areas of Food and Nutrition Security and Built Environment, to ensure that we and our partners have a clear path forward for determining community-driven next steps to achieve real change in food and housing security. The basic building blocks of health and stability - nutritious food and safe, affordable housing - are unfortunately difficult for Trenton residents to achieve and sustain. This reality underlies the stark differences in health outcomes, incidence of chronic disease, and socioeconomic outcomes for Trenton residents and those in the rest of Mercer County. By the end of the period of performance, THT will achieve two outcomes as outlined in the logic model for CDC-RFA-DP-23-0077. First, collaboration and engagement will be increased across multisectoral partners through the establishment of a Multi Sector Leadership Team. Second, an implementation-ready SDoH Accelerator plan will be developed, including all required components, by the end of the proposed funding cycle.
