Bridging the Divide: Building a Multi-Sector Health Equity Accelerator Plan for Social Determinants of Health in Greater Portland - The purpose of this project is to develop a comprehensive multisector Accelerator Plan to address Social Determinants of Health (SDOH) and will specifically focus on Community-Clinical Linkages and Social Connectedness among disparate populations in Greater Portland, Maine- specifically Portland, Westbrook, and South Portland. By understanding and addressing the identified social determinants, we can create a more equitable and healthier community. The project will involve a collaborative effort among key stakeholders, including but not limited to local government agencies, community organizations, healthcare, and academic institutions identified as the Portland Health Equity Stakeholder Alignment (PHESA). Using the existing six Community Health Improvement Plans, the next phase will focus on creating a SDOH Accelerator Plan that aligns with the needs of the community. This phase will involve designing and implementing evidence-based interventions that address community-clinical linkages and social connectedness. This may include initiatives such as improving access to care, strengthening access to preventive health services, promoting clinical fruit and vegetable treatment programs, integrating social support services and activities for older adults, and strengthening community support networks. The proposed interventions will be tailored to the unique needs and characteristics of the Greater Portland community and our target population of Black or African American individuals and Hispanic or Latino individuals, of any race. Throughout the project, an emphasis will be placed on fostering collaboration and partnerships among different sectors. By bringing together diverse stakeholders, including government, healthcare, education, and community-based organizations, a coordinated and integrated approach can be developed to address the social determinants of health effectively. Evaluation and monitoring will be integral components of the project. The impact and effectiveness of the interventions will be assessed through quantitative and qualitative measures. This will help in refining strategies, identifying areas of success, and making adjustments as needed. Ultimately, the SDOH Accelerator Plan developed through this project will provide a roadmap for improving the social determinants of health in Greater Portland, ME, with the goal of creating a healthier, more equitable, and thriving community for all residents. It will serve as a model for other communities seeking to address social determinants and promote population health.
