Designing an SDOH Accelerator Plan for Two Providence Health Equity Zones using the Pathways to Population Health Equity Framework - The Executive Office of Health and Human Services, in partnership with the Rhode Island Department of Health (RIDOH), will convene a leadership team from Care Transformation Collaborative of Rhode Island (CTC-RI), Rhode Island Medicaid, Managed Care Organization (s), place-based Community Health Workers, Providence Community Health Centers (FQHC) and Two, Providence-based Health Equity Zones: 02907 and South Providence (02905), to design an SDOH accelerator plan. The purpose of this accelerator planning process is to develop an implementation-ready Accelerator Plan guided by the Pathways to Population Health Equity (P2PHE) framework. P2PHE will support Rhode Island in bringing together clinical and community partners to develop a balanced approach to SDOH interventions at the systems, community and individual level. The SDOH domains we will focus on are social connectedness, food and nutrition security and clinical-community linkages informed by existing state and community level data in addition to HEZ resident priorities. The outcomes to be achieved during the project period include: 1) Increased collaboration and engagement across multisectoral partners; 2) Completed implementation ready SDOH Accelerator Plan, including all required components. The longer-term goal is to reduce geographic, racial and socio-economic disparities in chronic diseases, such as diabetes and asthma and the disparities in associated risk factors such as obesity.
