Illinois WISEWOMAN Program - The overall goal of the Illinois WISEWOMAN Program (IWP) is 1) to improve access to cardiovascular disease (CVD) related preventive health services, including blood pressure and cholesterol screenings, and 2) to lower the risk for heart disease through risk reduction counseling based on screening results, health coaching, and referral to one or more appropriate community-based healthy behavior support options (HBSS). IWP will partner with seven Illinois Breast and Cervical Cancer Program providers to serve the target population, women who are 35-64 years of age, living at or below 250% of the federal poverty level, are uninsured or under-insured, and are currently living in a county served by IWP, and will recruit two new providers. HBSS options available to IWP participants include Weight Watchers, Taking Off Pounds Sensibly (TOPS), NOOM, Self-Monitoring Blood Pressure Program (SMBP), Wise Words Text Message Program, and Expanded Health Coaching with Community Supports.
The IWP will continue its partnership with the University of Illinois College of Medicine at Rockford for program evaluation, which will include both process and outcome evaluation.
