CT Department of Public Health WISEWOMAN Program - The Connecticut Department of Public Health (DPH) is requesting $675,460 from the Centers for Disease Control and Prevention to continue and expand current efforts in developing comprehensive and collaborative approaches using evidence-based interventions decrease cardiovascular (CVD) risk and burden. These outcomes will be achieved by increasing appropriate screening services through provision of assessment of cardiovascular risk factors, eliminating barriers, and implementing key evidence-based strategies; supporting state-wide coalitions and plans to inform strategic policy, systems, and environmental changes. DPH proposes to apply for the DP23-0003 to accomplish the above.
The Connecticut Well Integrated Screening and Evaluation for Women Across the Nation (WWP) proposes to provide timely and appropriate heart disease risks assessment and social services referral to uninsured or underinsured eligible women through a network of contracted health care and social services agencies throughout Connecticut. In addition, it proposes to provide timely and appropriate patient navigation services and bilateral referral to support low-income women through assessing and addressing social needs and structural barriers to preventive screening and diagnosis of heart disease. The WWP proposes to provide services to reduce those risks through improved diet, physical activity, tobacco cessation, and medication adherence support. The WWP also proposes to increase population-based screenings through partnerships and collaborations with community-based organizations, professional associations, large health systems, and employers to educate, promote awareness, refer to social services and reduce structural barriers preventing women from accessing services.
The Connecticut WWP proposes tocontinue to use a collaborative approach to implement healthy behavior support services options that reduce the risk of cardiovascular diseases in Connecticut communities. This collaboration between public health, communities and health care systems will provide a framework to improve the scope and quality of efforts to reduce disparities and promote equitable health care services.
