Promoting Healthy Heart for Women: A Community Based Approach and Increasing Access to Preventive Health Services for Underserved Women - The primary purpose of the Well-Integrated Screening and Evaluation of Women Across the Nation (WISEWOMAN) grant is to improve cardiovascular health in a specific population with a focus on advancing health equity. It will support the implementation and evaluation of a core set of evidence-based strategies proven to reduce heart disease and stroke rates through early detection and treatment of hypertension and high cholesterol in low-income, uninsured, and underinsured participants aged 35 to 64. The Well-Integrated Screening and Evaluation of Women Across the Nation (WISEWOMAN) will integrate with the National Breast and Cervical Cancer Early Detection Program (NBCCEDP) to extend preventive health services to achieve optimal cardiovascular health for participants ages 35-64 who are participants of the CDC funded NBCCED. The program helps participants understand and reduce their risk of cardiovascular disease and benefit from early detection and treatment. With health equity as a guiding principle, WISEWOMAN provides culturally informed risk factor screenings and program services that are mindful of social determinants of health. Cardiovascular Disease (CVD), including heart disease, stroke, and other vascular diseases, is a major cause of morbidity and mortality in Delaware. In 2015, heart disease and stroke accounted for 22.6% and 5.4%, respectively, of all deaths statewide. To optimize CVD outcomes among vulnerable Delawarean women, the Screening for Life (SFL) program will collaborate with internal and external partners to extend cardiovascular health services to SLF-eligible low-income, uninsured, and underinsured Delawarean women aged 35-64. Service expansion includes CVD risk assessment by SFL providers, personalized risk reduction counseling, medical follow-up, and referrals to community-based resources.
