The Hawai‘i Department of Health, Executive Office on Aging (EOA) will implement the goals, objectives and strategies developed in the Hawai‘i 2035: State Strategic Plan for ADRD to build a dementia care infrastructure and increase the state’s capacity to address ADRD by: 1) Maintaining and expanding an engaged BOLD Coalition;
2) Orienting coalition members and partners to brain health, ADRD issues and the broad spectrum of actions needed to fully address ADRD statewide;
3) Creating an implementation plan for the Hawai‘i 2035: State Strategic Plan for ADRD;
4) Educating the general public about ADRD topics and specifically populations of high burden such as Native Hawaiians, Filipinos, Japanese, Koreans and Micronesians, on risk reduction, early diagnosis of ADRD, prevention and management of comorbidities;
5) Educating providers and other professionals about ADRD topics, including dementia risk reduction, early detection and diagnosis, linkages to treatment, care, and services, and prevention and management of comorbidities and caregiving for persons with dementia;
6) Increasing the availability and use of data to improve knowledge and inform data driven
decision-making about Hawai‘i 2035: State Strategic Plan for ADRD goals and activities;
7) Improving sustainability of ADRD efforts in the Hawai‘i 2035: State Strategic Plan for ADRD;
8) Increasing the number of community-clinical linkages among health care systems and services, public health agencies, and community-based organizations.
