Centers for Disease Control and Prevention (CDC) BOLD Grant - Alzheimer's and Related Dementia (ARDR) - Allegheny County, Pennsylvania, is one of the oldest city regions in the United States, with 19.7% of the county population 65 years and older. Allegheny County has the second largest population of older adults among the 40 largest U.S. counties, second to Palm Beach, Florida. Allegheny County's population will likely age primarily in place. By 2050 the population aged 85+ is projected to increase by 27,000+ or over 85% compared to current levels. Based on the Community Health Assessment and review of community-level data gathered, ACHD will develop and implement a public health response to Alzheimer's disease and related dementias (ADRD) in Allegheny County. The Allegheny County BOLD Coalition (ABC) will develop and implement a multi-sector ADRD Strategic plan for the county that addresses the social determinants of health (SDOH). Implementation will focus on priority Black communities to address racial disparities on the impact of ADRD, which will coordinate with the CDC-funded Racial and Ethnic Approaches to Community Health (REACH) Coalition for Allegheny County. ACHD is applying for Component 1 in response to the notice of funding from the Centers for Disease Control (CDC). ACHD aims to build an inclusive and equitable dementia infrastructure in Allegheny County, and the ACHD Epidemiology department created a strategic evaluation plan. The work of the ABC will follow the conceptual framework of the Healthy Brain Initiative (HBI) Road Map informed by the four essential services of public health: assure a competent workforce, monitor, and evaluate, develop policies and mobilize partnerships and educate and empower the nation.
