BOLD Public Health Programs to Address Alzheimer's Disease and Related Dementia - The purpose of this grant proposal is for the California Department of Public Health (CDPH) Alzheimer’s Disease Program (ADP) to work with local health jurisdictions (LHJs) to build upon the existing public health infrastructure and implement evidenced-based community interventions to address the social determinants of health (SDOH) and reduce health disparities among disproportionately impacted priority populations living with Alzheimer’s disease and related dementias (ADRD). The proposed activities include building awareness of ADRD among the general public, increasing knowledge among healthcare providers, and increasing utilization of community resources and support services for ADRD at the local level. Utilizing a health equity lens for the proposed public health approach, collaboration with key partners include the Statewide Alzheimer’s Disease and Related Disorders Committee (coalition), California Alzheimer’s Disease Centers (CADCs), local health departments, and the California Department on Aging (CDA) for capacity building activities. The proposed strategic plan (California Master Plan on Aging) aligns with our implementation plan (Recommendations for Alzheimer’s Prevention, Preparedness, and the Path Forward) by focusing on activities that advance primary, secondary, and tertiary levels of ADRD prevention.
