Washington State Bold Grant Initiative - The Washington State Department of Health (DOH), the Washington State Dementia Action Collaborative (DAC), and our many dynamic partners seek to implement Component 2 of the BOLD grant to bolster and broaden our work. Combing the goals, strategies, and activities of the Washington State Plan to Address Alzheimer’s Disease and Other Dementias with the BOLD work and implementation plans, will help us to impact the CDC’s six pillars of work to address social determinants of health, those pillars are: Policy and Law, Data and Surveillance, Evaluation and Evidence Building, Partnerships and Collaboration, Community Engagement, and Infrastructure and Capacity, with Equity as the core.
