New Hampshire is applying for Component B funding to support expansion of current infrastructure in place to support the dissemination of several Arthritis-Appropriate Evidence-Based Interventions (AAEBIs) and to expand on established mechanisms in place supporting physical activity counseling for arthritis management and referrals to AAEBIs. Strategies will be implemented statewide with a focus on underserved populations who experience disproportionate burden of arthritis including people living in poverty, people living in rural communities, and people with diabetes. The following outcomes will be achieved through this project: 1) increased AAEBI enrollment; 2) increased percent of adults with arthritis who get healthcare counseling about physical activity and arthritis management; and 3) reduced disparities in arthritis management and outcomes among adults with arthritis. Through extensive strategic partnerships, experience with all proposed strategies, and staff with experience and subject matter expertise in diabetes prevention & management, the organization has the capability to serve all populations and communities in NH that the Program is proposing to work with. All work proposed with be data-driven and evaluation and performance measurement will be ongoing throughout the project including use of BRFSS for measuring outcomes.
