The California Department of Public Health (CDPH) is a state health department applying for the Centers for Disease Control and Prevention funding opportunity, CDC-RFA-DP23-0004, National Center for Chronic Disease Prevention and Health Promotion, The National Cardiovascular Health Program. CDPH has the experience, tools, commitment, partnerships, and resources needed to serve all Californian populations and communities in this grant.
This funding will support public health efforts in the prevention and management of cardiovascular disease. Program strategies include tracking and monitoring clinical and social services and support, implementing team-based care, and linking community resources and clinical services to prevent and manage hypertension and high blood cholesterol in California. CDPH will integrate and implement health equity solutions to address the needs of populations that are disproportionately underserved, uninsured, and underinsured with or at risk of hypertension and high blood cholesterol. The priority populations include African American, Asian American, American Indian or Alaska Native, and Hispanic communities.
CDPH will collaborate with other state agencies, learning collaboratives, local health departments, health systems, and partners, focusing on health equity and Social Determinants of Health, which will result in better health outcomes. CDPH will access community resources and foster sustainability through working with partners across the state, reducing disparities in blood pressure control and high blood cholesterol, and increasing the use of social services supports among populations at highest risk of cardiovascular disease.
