Project Abstract Summary Objective: Well-Ahead Louisiana (WAL) will coordinate efforts within the State of Louisiana to leverage resources and maximize reach to address the need to improve cardiovascular health and reduce health disparities in target populations with the highest risk of cardiovascular disease. This will be achieved through the utilization of the Population Health Cohort, Community-based Self-Monitored Blood Pressure programs and the implementation of the Heart Disease Learning Collaborative. Project Purpose: WAL proposes to implement strategies through a tiered approach with mutually reinforcing statewide and regionally targeted strategies. Statewide work focuses on increasing the awareness of heart disease, high blood pressure, and high cholesterol among populations with the highest risk, along with an increased understanding of the risk factors and resources available to prevent the onset of cardiovascular disease. WAL proposes to further focus on provider education and partnership with health systems, payers, and other public health partners to increase the reach and utilization of programs and best practices to improve outcomes related to cardiovascular disease through the implementation of the Heart Disease Learning Collaborative. Regionally, WAL has identified six Well-Ahead Chronic Disease Priority Regions where WAL will target individual health systems, providers, and community organizations to launch new programs, establish and improve referral mechanisms, increase community-clinical linkages and conduct quality improvement initiatives. Outcomes: WAL expects to achieve the following outcomes: Improve blood pressure control among populations within partner health care and community settings; reduce disparities in hypertension control among populations within partner health care and community settings; increase utilization of social services and support among populations at highest risk of CVD, with a focus on hypertension and high cholesterol; improve cardiovascular health; and reduce disparities in cardiovascular health.
