The Delaware Division of Public Health’s Diabetes and Heart Disease Prevention and Control Program aims to aims to implement evidence-based strategies to improve the management and prevention of cardiovascular disease (CVD) in populations disproportionately at risk in Delaware. The project will focus on identifying and addressing disparities in CVD risk factors, such as hypertension, diabetes, and obesity, among priority populations, including racial and ethnic minorities, older adults, and individuals with low income or limited access to healthcare. The project will involve collaboration with healthcare providers, community-based organizations, and other stakeholders to identify and implement interventions that have been shown to be effective in reducing CVD risk and improving outcomes. The goal is to reduce the burden of CVD and improve health equity for all residents of Delaware.
Delaware will continue to build capacity and sustainability around Category B work for cardiovascular disease prevention and management strategies implemented during the 5-year funding cycle with 1815 as well as develop or join a learning collaborative.
During the first year of the DP-23-0004 grant, DPH will work diligently to address strategies and measures defined in the application.
• Track and Monitor Clinical and Social Services and Support Needs Measures Shown to Improve Health and Wellness, Health Care Quality, and Identify Patients at Highest Risk of Cardiovascular Disease (CVD) with a Focus on Hypertension and High Cholesterol.
• Implement Team-Based Care to Prevent and Reduce CVD Risk with a Focus on Hypertension and High Cholesterol Prevention, Detection, Control, and Management through the Mitigation of Social Support Barriers to Improve Outcomes.
• Link Community Resources and Clinical Services that Support Bidirectional Referrals, Self-Management, and Lifestyle Change to Address Social Determinants that Put the Priority Populations at Increased Risk for Cardiovascular Disease with a Focus on Hypertension and High Cholesterol.
Delaware has been successful in several areas of healthcare, population health and improving access to cardiovascular care in underserved areas. Building on its strong internal infrastructure and unique relationship a diverse set of partners, the Delaware Division of Public Health is prepared to initiate the development and implementation of the proposed initiatives.
