The New Mexico Department of Health (NMDOH), Heart Disease and Stroke Prevention Program (HDSPP) aims to address health disparities in cardiovascular disease (CVD) by reducing social determinants of health (SDOH) related barriers impacting priority populations at highest risk for CVD. New Mexico is prioritizing and emphasizing strategies to reach priority populations that will allow us to achieve statewide reach, inclusive of all populations and communities in the state because we believe statewide change happens when we address the communities that suffer the largest health burdens. New Mexico has identified the following as priority populations: Black/ African American, people experiencing poverty, and Chaves and Luna counties. We have chosen these populations based on the following factors related to disproportionate incidence and prevalence of CVD and hypertension.
