Washington State Department of Health (DOH), physically located at 111 Israel Rd SE, Tumwater WA 98501, proposes to implement and evaluate 9 evidence-based and best practice strategies throughout WA State in response to NOFO DP23-0020, A Strategic Approach to Advancing Health Equity for Priority Populations with or at Risk for Diabetes – Component A. While DOH has the capability to serve all populations and communities within our state, we will prioritize communities who have systematically experienced greater obstacles to health, resulting in higher rates of diabetes and related hospitalizations and deaths. Epidemiological data shows how social determinants of health such as race, ethnicity, geographic location, level of education, and income level can predict treatment and prevention outcomes among persons with diabetes or pre-diabetes, and that risk of diabetes is rising among youth. DOH will advance health equity by collaborating with organizations that have experience with our priority populations in determining feasible individual level evidence-based approaches to Type 2 diabetes prevention and management, as well as policy, system and environmental level interventions to address the social and economic conditions that affect diabetes-related health. DOH will partner with multi-sector stakeholders to advance equitable access to appropriate and feasible Diabetes Self-Management Education and Support and nationally recognized Diabetes Prevention Programs, building on investments DOH has made in support of Community Health Workers, care coordination and multi-disciplinary team-based care. As a result, we anticipate seeing improvements in immediate and long-term health outcomes for WA residents, including the identified priority populations, within the five-year timeline.
