A Strategic Approach to Advancing Health Equity for Priority Populations with or at Risk for Diabetes - The South Dakota Department of Health (SD DOH), a state health department, is applying for funding under Component A of CDC-RFA-DP23-0020—A Strategic Approach to Advancing Health Equity for Priority Populations with or at Risk for Diabetes. The proposed area of focus is the entire State of South Dakota, for a total statewide population reach of 895,376 people. The physical address of SD DOH is 600 East Capitol Avenue, Pierre, SD 57501-2536. SD DOH has a strong history of serving the disease prevention and health promotion needs of ALL populations and communities in South Dakota for several decades. Through the Diabetes Program (DP), the SD DOH will implement and evaluate the purpose of the proposed project, which is to decrease the risk for type 2 diabetes among adults with prediabetes and improve self-care practices, quality of care, and early detection of complications among people with diabetes. The project approach will consist of evidence-based diabetes management and type 2 diabetes prevention and risk mitigation strategies, with an emphasis on reducing health disparities for priority populations. SD DOH has selected six (6) DP23-0020 strategies, that include SMARTIE (e.g., specific, measurable, attainable, relevant, timebound, inclusive, equitable) Objectives written in the DP23-0020 Work Plan template. The proposed project will be evaluated, and the data collected will be appropriately managed. Preliminary details surrounding the Evaluation and Performance Measurement Plan (EPMP) and the Data Management Plan (DMP) are described herein, with additional detail provided six (6) months post-award. Under this project, SD DOH adopts all Performance Measures and all Short, Intermediate, and Long-Term Outcomes listed under Component A in DP23-0020, without modification. SD DOH will build upon the collaborative work achieved through prior Centers for Disease Control and Prevention (CDC) funding opportunities (i.e., 1305, 1815). Internal and external partners work across SD DOH programs to leverage resources and relationships that will produce the greatest public health impact and reach of DP activities. SD DOH will maximize partnerships with healthcare systems, tribal organizations, quality improvement organizations, and academic institutions to implement the evidence-based strategies of DP23-0020. The strong partnership with the South Dakota State University College of Pharmacy, Community Pharmacy Enhanced Services Network (CPESN), South Dakota State University Extension Better Choices, Better Health SD® (BCBH-SD), Community Health Worker Collaborative of South Dakota (CHWSD), South Dakota Foundation for Medical Care (SDFMC), South Dakota Diabetes Coalition (SDDC) and Community HealthCare Association of the Dakotas (CHAD) will provide a broad statewide base to reach high burden and priority populations through utilization of evidence-based practices and training.
