A Strategic Approach to Advancing Health Equity for Priority Populations with or at Risk for Diabetes - Indiana Project Abstract Summary The Indiana Department of Health (IDOH) Division of Chronic Disease, Primary Care and Rural Health is applying for the Centers for Disease Control and Prevention (CDC) CDC-RFA-DP23-0020 - A Strategic Approach to Advancing Health Equity for Priority Populations with or at Risk for Diabetes – Component A. This funding opportunity will support a coordinated effort across IDOH program areas to improve the health of Indiana’s residents in the communities with the highest burden for diabetes and for those at highest risk for diabetes. IDOH serves all citizens and communities within Indiana. Activities proposed by IDOH will target those Indiana groups that are disproportionally impacted by diabetes and prediabetes. To achieve outcomes, ISDH will implement eight strategies within Component A to implement a comprehensive approach to diabetes services and management. Many of the targeted communities and counties have limited community resources for diabetes self-management education and support programs, limited to no access for the National Diabetes Prevention Program and to community health workers. Strategies for developing community resources and to connect communities for additional diabetes access will allow a comprehensive implementation of strategies in health systems and community-based organizations for Component A. IDOH will also work directly with the health systems and community-based organizations to increase the utilization of community health workers for diabetes and prediabetes. Sustainability activities for the CHWs will be addressed at the state-level for policy development, Medicaid reimbursement and a process for education and certification.
